The names of those who you wish to have remembered at the Annual Association memorial Service during the Summer Education Conference, must be submitted no later than June 10 Th. The person you wish remembered does not have to have been a member of the Association. They only need to have been associated with a public safety services agency in one way or another. Police, fire and EMS providers are the ones we think of first, but dispatchers, department supporters and trustees, or just about anybody who may have been important to your organization can have their name submitted for remembrance.

Do not wait until the Annual Summer Conference contact OAEMS.

Please print and fill out the below form and return it to OAEMS by mail or fax. Memorial Service Committee will make every effort to notify the persons family and request their attendance/participation in the services.
